That can definitely be the result of acid reflux and it can persist especially if she is not on the correct medication specifically for her. I would recommend taking her to a pediatric gastro specialist significant reflux can cause serious issues later on if left untreated. Hope this helps I have alot of experience with patients with acid reflux not as much in pediatrics but we have referred children to specialists for similar cases. Good Luck.
Hi, 4y old with 2mo history of vomiting with tentative diagnosis of GERD and is being treated for the same. Main medications used are prokinetic drugs which tend to increase gastric tract motility to make the food go down faster (metoclopromide); then there are anti-secretory drugs which include H2blockers (ranitidine) to decrease production of acid and Proton-pump inhibitors (omeprazole) which tend to decrease secretion of acid. You did not mention the medication so I presume it is one of the later two because these are the most prescribed ones. GERD is produce by problem with lower esophageal sphincter. Some times medications need some time to work or other they might need dosage adjustment as the diet and food pattern changes. Remember emotions can produce acid too. Now coming back to the problem – only in the mornings on empty stomach and not other times…uhm. Anything she eats or drinks before she goes to the bed that she doesn’ t eat or drink at other times (or the amount of so)? You may want to add a pillow (one or two at most) for gravity to help. Check the vomit – how does the food looks? If it looks like what she had that means it did not go into the stomach at all – in that case motility drugs may make more sense than acid reducing drugs. Check her stool for any black spots (blood) – if you see those let your Dr. know – then she needs acid drugs and may be some antibiotics. It could be that the sphincter has shrunk which doesn’t make sense as she has been okay for 4y, but sphincter is just a muscle and things can happen. You did the right thing by taking her to the Dr. Prolonged vomiting is not good esp if it is coming form the stomach – it can change the wall structure of the esophagus which can create other problems. Hope this helped. I will look around some and if find some additional notes i will re-post. Take care and hope she starts to feeling better.
